FIRMAGON REIMBURSEMENT CODES & MONTHLY BILLING
FIRMAGON Is Billed By The Milligram (mg): Starting Dose = 240 mg; Each Subsequent Dose = 80 mg.

99213: A level-3 established office visit involves 2 out of 3 of the following: expanded problem-focused history, expanded problem-focused examination, and medical decision-making of low complexity or 15 minutes of face-to-face with the patient.
